How To Cancel
We want your experience with us to be seamless and stress-free. Below is a simple overview of how cancellations and refunds work for prepaid memberships.
3-Day Rescission Period
If you change your mind within 3 business days of signing your agreement, you may cancel for any reason.
To rescind, please submit a written notice in person or send it by certified or registered mail to:

Cancellation After 3 Days
After the rescission window closes, cancellations are only allowed in the following situations:
1. Permanent Disability or Death
If a permanent disability prevents you from using the services, or in the event of death, the membership may be cancelled without penalty. A physician may be required to verify the disability. Any prepaid amounts will be refunded pro rata, minus the value of services already used.
2. Moving More Than 25 Miles Away
If you relocate more than 25 miles from the facility and we cannot transfer your membership to a comparable location near your new address, you may cancel and receive a pro rata refund for unused services.
Prepaid Membership Terms
Prepaid memberships cannot be cancelled early unless they qualify under the reasons listed above. The membership will end automatically at the end of the Initial Term.
To avoid a lapse in access, please contact a [locationname] sales manager at least 30 days before your term ends.
Refunds
Approved refunds (for valid rescission or qualifying cancellations) will be issued for unused prepaid amounts, minus the value of services already received. Refunds may be sent electronically to the card or account on file, according to state law timelines.
