I agree to pay PickUp USA Fitness - Irwindale (hereinafter referred to as "Gym") $49.99 per month. I understand that membership will remain active, and Gym will continue automatic monthly deductions from my designated payment source, in the amount of $49.99 per month, unless written notice is provided to Gym instructing the termination of automatic payments as detailed below.

I understand that I may cancel this agreement at any time without penalty. To cancel, I must go to the location where I originally enrolled and sign required electronic cancellation form. Automatic payments will terminate once I sign cancellation form.
